Job Description

Occupational Therapist Join a friendly and supportive multidisciplinary team in North Coast NSW and be responsible for improving older people's independence and confidence after a hospital stay. Transition care is goal-orientated, time-limited and therapy-focused. It provides older people after a hospital stay with a package of service that includes low intensity therapy. Services will include: *Comprehensive assessments and therapy planning *Clinical observations *Equipment prescriptions, home modifications and training for patients and families *Upper limb therapy You will be helping older people complete their restorative process and optimise their functional capacity, while assisting them and their family to make long-term care arrangements. The successful candidate will be joining a well established facility and working closely with the allied health and medical teams. *Relevant qualifications in Occupational Therapy *2+ years local post graduate experience *Experience working in aged care
